New Patient Information
Thank you for your interest in my practice. I look forward to helping you achieve and maintain good health through Integrative and Functional Medicine. To avoid any confusion about the services I offer, I require all new patents (or their legal representative) to review and acknowledge my practice policies and disclosures below:
I am Board Certified in Internal Medicine; however, I practice Integrative and Functional Medicine. This approach, which aims to identify and treat root causes of disease, often differs from the conventional “standard of care” for many conditions. For this reason, physicians who are not trained in Integrative, Functional or Environmental Medicine approaches may disagree with and/or disparage the treatment plan because they are unfamiliar with the science to support interventions such as: functional lab testing, as comprehensive stool analysis or hair/urine testing for heavy metals/chemical toxins, food sensitivity testing, environmental home assessments/mitigation for mold or electromagnetic fields, homeopathy, and other integrative approaches.
Working with an Integrative/Functional physician means partnering to co-create health and understanding, with some exceptions, that recovering from chronic illness generally takes a long time. It also requires hard work on your part. All treatment plans are personalized, but as a general idea they may include dietary modifications such as removing gluten, dairy, sugar and ensuring an organic/GMO free diet, dietary supplements, lifestyle modifications to optimize sleep/wake cycles, and environmental modifications to decrease your exposure to everyday toxins. Functional lab testing is ordered to inform and guide the treatment plan.
I do not accept any commercial insurance, Medicare or Medicaid. Payment for professional services is due at the time of service and is billed at the hourly rate of $300.00. Initial consultation fees for in-person visits range from $375-$450 (75-90 minutes, depending on complexity). Initial visits done by telehealth range from $300-375 (60-75 minutes). Follow up visits are also billed by time, at the hourly rate and vary in length from 30-60 minutes. For house calls there is a $50 surcharge for travel, in addition to the hourly rate.
I will not be billing your insurance company. Instead, I will give you a superbill that you can submit to your insurance company. I am considered an out of network provider and reimbursement, if any, will depend on the out of network benefits of your particular health plan as well as how your insurance reimburses for the services provided.
Superbills are provided upon request, after payment is received, for patients who are reside in New Mexico or Florida – the states where I am actively licensed. To determine if you are eligible for out of network provider reimbursement, contact your insurance carrier directly.
Additional fees that you will incur include:
- Lab fees for functional lab work (you pay the direct/prompt payment price, directly to the lab).
- Supplements are sold via virtual dispensary (Fullscript), discounted 25% off retail price.
- For extremely complex cases where extensive chart review is required, an additional charge may apply for time spent reviewing records if the required time is greater than 30 minutes. You will be notified, in advance, if this is the case and this does not apply to most patients.
If you need to cancel or reschedule an appointment, a minimum of 2 business days’ notice is required, so for a Monday appointment, we need to hear from you by Thursday. For the first last minute cancellation or missed appointment, you will be charged $50 dollars. If it happens a second time you will be charged $100.
My Integrative/Functional Medicine practice focuses on improving your overall health by identifying symptoms of imbalance, such as fatigue, anxiety, sleep issues, constipation, chemical sensitivities, etc., and recommending lifestyle, dietary and supplementary interventions to help restore balance and optimize your health. As an Integrative/ Functional Medicine practitioner, I will:
- Obtain a detailed history and conduct a thorough physical examination.
- Create a timeline to document health challenges and identify symptom complexities that point to areas of imbalance.
- Establish targeted goals to help restore balance and optimize health.
- Recommend personalized dietary changes, targeted supplementation, lifestyle and environmental modifications.
Functional medicine works in tandem with and is not a replacement for any care or treatment that you receive from your primary care or specialist physicians. I will not be your primary care provider and recommend that you discuss your functional medical treatments, dietary changes, and/or supplements with your primary care physician to ensure continuity of care.
Electronic Health Records
Your medical records will be maintained on a secured and encrypted HIPAA compliant electronic health record (“EHR”) system maintained by Power2Practice. As a patient of my practice, you will have the opportunity to sign-up for and utilize your own patient portal on the Power2Practice website which will allow you to securely access your medical records, request appointments, communicate with me, and complete patient questionnaires at any time.
In keeping with patient privacy laws and HIPAA, I encourage all patients to communicate with my office via the secure patient portal messaging system. This messaging system allows you to communicate securely with me, request appointments, and gives you direct access to your protected health information.
